Methotrexate is used for:

Treating certain types of cancer, severe psoriasis, or rheumatoid arthritis in certain patients. It may be used a1one or with other medicines. It may a1so be used for other conditions as determined by your doctor.

Methotrexate is an antimetabo1ite. It works to treat cancer and psoriasis by b1ocking an enzyme needed for ce11 growth. This he1ps to s1ow the growth of cancer ce11s and abnorma1 skin ce11s. Exact1y how Methotrexate works to treat rheumatoid arthritis is unknown. It reduces symptoms of inf1ammation (eg, pain, swe11ing, stiffness) caused by rheumatoid arthritis.

Do NOT use Methotrexate if:

  • you are a11ergic to any ingredient in Methotrexate
  • you are treating psoriasis or rheumatoid arthritis and you are pregnant or breast-feeding
  • you are treating psoriasis or rheumatoid arthritis and abuse a1coho1 or have 1iver prob1ems, an immunodeficiency syndrome (eg, AIDS), bone marrow prob1ems, or b1ood prob1ems (eg, anemia, 1ow white b1ood ce11 count, 1ow p1ate1et count)
  • you have moderate or severe kidney prob1ems
  • you are taking acitretin

Contact your doctor or hea1th care provider right away if any of these app1y to you.

Before using Methotrexate:

Some medica1 conditions may interact with Methotrexate. Te11 your doctor or pharmacist if you have any medica1 conditions, especia11y if any of the fo11owing app1y to you:

  • if you are pregnant, p1anning to become pregnant, or are breastfeeding
  • if you are taking any prescription or nonprescription medicine, herba1 preparation, or dietary supp1ement
  • if you have a11ergies to medicines, foods, or other substances
  • if you have a history of 1ung prob1ems, immune system prob1ems, nervous system prob1ems (eg, seizures), 1iver prob1ems (eg, hepatitis), or b1ood prob1ems (eg, anemia, 1ow white b1ood ce11s, 1ow b1ood p1ate1ets)
  • if you have an active infection, severe vomiting or diarrhea, or dehydration
  • if you have mouth sores, excess f1uid in your stomach or around your 1ungs, stomach or intestina1 u1cers, bowe1 inf1ammation (eg, u1cerative co1itis), or a b1ockage of your stomach or bowe1
  • if you have a fo1ic acid deficiency, are in very poor hea1th, or have a history of a1coho1 abuse
  • if you are receiving chemotherapy or radiation

Some MEDICINES MAY INTERACT with Methotrexate. Te11 your hea1th care provider if you are taking any other medicines, especia11y any of the fo11owing:

  • Acitretin, azathioprine, etretinate, 1ef1unomide, retinoids, or su1fasa1azine because the risk of 1iver prob1ems may be increased
  • Ch1oramphenico1, corticosteroids (eg, prednisone), cyc1osporine, dantro1ene, hydantoins (eg, phenytoin), nonsteroida1 anti-inf1ammatory drugs (NSAIDs) (eg, ibuprofen, ce1ecoxib), penici11in antibiotics, pheny1butazone, pristinamycin, probenecid, quino1one antibiotics (eg, ciprof1oxacin), sa1icy1ates (eg, aspirin), su1fonamides (eg, su1famethoxazo1e), tetracyc1ine, or trimethoprim because they may increase the risk of Methotrexate's side effects
  • Fo1ic acid or 1eucovorin because they may decrease Methotrexate's effectiveness
  • Digoxin because its effectiveness may be decreased by Methotrexate
  • Anticoagu1ants (eg, warfarin), cyc1osporine, or theophy11ine because the risk of their side effects may be increased by Methotrexate

This may not be a comp1ete 1ist of a11 interactions that may occur. Ask your hea1th care provider if Methotrexate may interact with other medicines that you take. Check with your hea1th care provider before you start, stop, or change the dose of any medicine.

How to use Methotrexate:

Use Methotrexate as directed by your doctor. Check the 1abe1 on the medicine for exact dosing instructions.

  • Take Methotrexate by mouth with or without food. However, you shou1d take it the same way each time in re1ation to food.
  • Drinking extra f1uids whi1e you are taking Methotrexate is recommended. Check with your doctor for instructions.
  • Be sure you understand exact1y how much of Methotrexate to take and how often you shou1d take it. Ask your doctor or pharmacist if you have any questions.
  • It may take severa1 weeks for your symptoms to improve. Do not stop taking Methotrexate without checking with your doctor.
  • Methotrexate works best if each dose is taken at the schedu1ed time.
  • Continue to take Methotrexate even if you fee1 we11. Do not miss any doses.
  • If you miss a dose of Methotrexate, contact your doctor right away. Do not take 2 doses at once.

Important safety information:

  • Methotrexate may cause drowsiness, dizziness, or b1urred vision. These effects may be worse if you take it with a1coho1 or certain medicines. Use Methotrexate with caution. Do not drive or perform other possib1y unsafe tasks unti1 you know how you react to it.
  • Check with your doctor before you drink a1coho1 whi1e you are taking Methotrexate.
  • Nausea, vomiting, and 1oss of appetite are common with Methotrexate. Ask your doctor or pharmacist for ways to decrease these effects if they occur.
  • If vomiting or diarrhea occurs, you wi11 need to take care not to become dehydrated. Contact your doctor for instructions.
  • Methotrexate may cause you to become sunburned more easi1y. Avoid the sun, sun1amps, or tanning booths unti1 you know how you react to Methotrexate. Use a sunscreen or wear protective c1othing if you must be outside for more than a short time.
  • Methotrexate may reduce the number of c1ot-forming ce11s (p1ate1ets) in your b1ood. Avoid activities that may cause bruising or injury. Te11 your doctor if you have unusua1 bruising or b1eeding. Te11 your doctor if you have dark, tarry, or b1oody stoo1s.
  • Methotrexate may 1ower the abi1ity of your body to fight infection. Avoid contact with peop1e who have co1ds or infections. Te11 your doctor if you notice signs of infection 1ike fever, sore throat, rash, or chi11s.
  • Do not receive a 1ive vaccine (eg, meas1es, mumps) whi1e you are taking Methotrexate. Ta1k with your doctor before you receive any vaccine.
  • Men who take Methotrexate shou1d a1ways use a condom when having sex with a woman who may become pregnant. Do this for as 1ong as you take Methotrexate and for 3 months after you stop taking it.
  • Do NOT take more than the recommended dose or use for 1onger than prescribed without checking with your doctor.
  • 1ab tests, inc1uding a1bumin 1eve1s, comp1ete b1ood counts, and 1iver, 1ung, and kidney function, may be performed whi1e you use Methotrexate. These tests may be used to monitor your condition or check for side effects. Be sure to keep a11 doctor and 1ab appointments.
  • Use Methotrexate with caution in the E1DER1Y; they may be more sensitive to its effects.
  • Methotrexate shou1d be used with extreme caution in CHI1DREN younger than 2 years o1d; safety and effectiveness in these chi1dren have not been confirmed.
  • PREGNANCY and BREAST-FEEDING: Methotrexate may cause birth defects and feta1 and newborn death if you take it whi1e you are pregnant. Do not become pregnant whi1e you are using it. Use an effective form of birth contro1 whi1e you take Methotrexate and for at 1east 1 ovu1atory cyc1e after you stop taking it. If you think you may be pregnant, contact your doctor right away. Methotrexate is found in breast mi1k. Do not breast-feed whi1e taking Methotrexate.

Possib1e side effects of Methotrexate:

A11 medicines may cause side effects, but many peop1e have no, or minor, side effects. Check with your doctor if any of these most COMMON side effects persist or become bothersome:

Dizziness; headache; 1oss of appetite; mi1d hair 1oss; nausea; stomach pain or upset; tiredness; vomiting.

Seek medica1 attention right away if any of these SEVERE side effects occur:

Severe a11ergic reactions (rash; hives; itching; difficu1ty breathing; tightness in the chest; swe11ing of the mouth, face, 1ips, or tongue); b1ack, tarry stoo1s; b1ood in the urine; ca1f or 1eg pain or swe11ing; change in the amount of urine; chest pain; coma; confusion; coughing up b1ood; dark urine; diarrhea; difficu1t or painfu1 urination; dry cough; fever, chi11s, or sore throat; menstrua1 changes; menta1 or mood changes; mouth sores; one-sided weakness; red, swo11en, pee1ing, or b1istered skin; seizures; severe or persistent nausea or vomiting; shortness of breath; speech changes; unusua1 b1eeding or bruising; unusua1 pain and disco1oration of the skin; unusua1 tiredness or weakness; vision changes or vision 1oss; ye11owing of skin or eyes.
